Information required to complete this form can be found in the Training Plan or Course Schedule for your program. (See your Department Head/Coordinator for details).

Please complete one form for each course. Your shell will be created within two working days. You and your Department Head/Coordinator will be notified via email when your course shell is ready.

Please note: If requesting a shell for anything other than an actual course, please use 'N/A' in the required fields and provide an explanation in the Additional Comments field, at the bottom of the form.

IMPORTANT NOTE: Any user/teacher listed above MUST have VCC network access (myVCC) AND MUST login to Moodle at least once before an administrator can assign him or her to a course. (The first Moodle login creates a Moodle account for the user.) If he or she does not yet have access to myVCC, you may still request the course shell but the user will have to be added at a later date.
